Your student is enrolled in a school participating in the TRIO Educational Talent Search (ETS) College Bound Program, a federally funded program provided by Salt Lake Community College ("SLCC TRIO"). In order to provide effective services and meet federal grant reporting requirements, SLCC TRIO is requesting access to certain student education records, as described below.
1. Records Requested. SLCC TRIO is requesting access to the following categories of your student's education records: student and parent or guardian directory information; grades and progress reports; attendance records; class schedules; graduation and registration records; and discipline records.
2. Your Rights Under FERPA. The Family Educational Rights and Privacy Act ("FERPA"), 20 U.S.C. § 1232g, protects the privacy of student education records. SLCC TRIO staff are designated as school officials with a legitimate educational interest under FERPA for most record categories. This form requests your express authorization for access to discipline records and confirms your consent to the full scope of data access described above. You may revoke this authorization at any time by written notice to SLCC TRIO. Revocation will not affect records already accessed prior to revocation. Students who are 18 years of age or older, or who are enrolled in postsecondary courses, are eligible students under FERPA and control their own records.
3. Purpose. Records will be used solely to evaluate student success and needs, tailor program services, assess program impact, meet TRIO ETS federal grant reporting requirements, and connect students and families to additional resources. Records will not be used for any other purpose, shared with unauthorized third parties, or used for commercial purposes.
