 This opportunity is only for women I have NOT serviced before OR have not seen within the last year. Completion of this form does not guarantee you will be chosen. 

Each hair color service includes a trim or haircut, a deep steam, and a K18 restorative treatment. A full color + cut transformation typically starts at $650+ depending on how in depth the service is. Please note that as a "model", you will receive up to 50% off this service and your fee of $350 is due in FULL in order to confirm the booking.

If chosen, you will be contacted via text and email with your link to make your payment. Once received, your appointment will be made and your confirmation will be emailed to you. If any additional questions are needed to be asked, we will set up a virtual consultation to ensure we are both on the same page about your goals. 

Photo/ Video Release 

As a hair color model, I agree to being filmed for social media and/ or educational purposes to be placed inside of Hair Color Concepts® Academy. 

I give Adrienne Small and Hair Color Concepts, Inc the absolute right and permission to take, publish, reproduce, exhibit, project, display and/ or copyright photographic images, video or pictures of my hair ( my face does not have to be included if I verbally inform Adrienne Small for it to not be) whether still, single, multiple, or moving, in color or otherwise, through any form of media (print, digital, electronic, broadcast or otherwise), for advertising, marketing, publicity, educational, archival, or any lawful purpose. 

I waive any right that I may to inspect and approve the finished product that may be used or to which it may be applied now and/ or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising to the use of the image or product.

I release and agree to hold harmless Adrienne Small and Hair Color Concepts, Inc of and from any liability by virtue of taking pictures or using testimonial data inany processing towards the copletion of the finished product and any use whatsoever of such pictures or videos whether intentional or otherwise.

Release of Liability

By undergoing chemical services of any kind, including the use of (but not limited to) hair color, permanent, semi permanent, demi permanent or lighteners (bleach), I understand that there is a risk of reduced curl elasticity and my natural curls may stretch/loosen during this process.

As my stylist, Adrienne Small agrees to take every precaution possible to maintain the integrity of my hair. However, it is important that I am made aware of this possibility and upon leaving the salon, I am accountable for maintaining it's integrity by following the steps, guidelines, and product usage presented to me during my appointment.

Should I NOT follow the given steps and guidelines and my hair is affected in a negative way, I release Adrienne Small and Hair Color Concepts, Inc of all liability, claims and demands; that this release contains the entire agreement between me as the client and Adrienne Small as the stylist; and that the terms of this release are contractual and not merely a recital.
