Recurring Appointment Request*

I, {cardholderName}, as the cardholder, hereby authorize Joy Music Studios LLC to charge my credit card for monthly music lessons starting September 1, 2024 for a 10 month payment plan ending on June 30, 2025. I confirm that the information for the credit card and billing address is complete and accurate.

I have been informed that I can cancel the recurring payment at least 15 days before the payment by written notice sent to info@joymusicstudios.com.

Discontinuation OR Change in Reoccurring Classes: If a student decides to discontinue classes, or there is a change in reoccurring classes, a two week paid notice, in writing is required. NO EXCEPTIONS.*

Missed Classes and Make-Up Policy: There are NO refunds given for missed classes WITHOUT a 24 hr notice of scheduled time of class in advance, NO EXCEPTIONS. If you have to miss a class due to ANY reason please call or text your teacher within 24 hours of your scheduled class time and let them know the reason. You will be offered a make up class according to your teacher’s availability.*

Forms of Payment: All students are required to have a credit card on file. The card on file will be charged for your weekly classes on the 1st of every month, if a holiday falls on your class day, the teacher will inform of an alternative class day for that class. In this occurance, this class would not count as one of the 2 additional make up classes offered within each semester.*

If any credit cards come back as failed payment and it is not resolved by the end of the first day of the month, there will be a $30.00 card processing error fee added in addition to that months class charge.*

PR Release, Hold Harmless Form This is giving permission for your student to be a part of recital videos, performances, and allowing their videos to be posted on studio websites and social media. I give permission to Joy Music Studios to photograph, record or video I myself or my child(ren) during the course of taking lessons to use the photos or video footage for the purpose of promotion on Joy Music Studios music pages, archiving and training.

By signing below I also hold Alecia Fiechter and Joy Music Studios LLC and it's endeavors and business harmless from ALL liability at any time, before, during or after lessons, whether they are a student or not. I release Alecia Fiechter and any of the Joy Music Studio representatives and teachers from all liability, costs, and damages that might occur during a participation in all events or activities. If emergency treatment for a student is necessary, I provide consent for Joy Music Studios to seek out emergency treatment if my emergency contact cannot be reached. I agree to accept financial responsibility for the costs relating to any emergency treatment given to myself and/or my child(ren).

Further I have read and agree to ALL policies in this form as long as I am participating in ANY lesson or activities with Joy Music Studios. This authority remains in effect as long as I myself and/or my child(ren) are taking lessons or involved in any special programs including recording sessions at Joy Music Studios.

If you have to miss a class due to ANY reason please call or text your teacher with Joy Music Studios within 24 hours of your scheduled class time. Your teacher will offer 2 make up class times every 2 months to make up a class you may have missed. If you are absent more than 2 classes in a semester, you understand 3 absences exeeds the limit of make up classes available, and the 3rd missed class would be a forfeited class. Forms of Payment: All students are required to have a credit card on file. This card will be charged on the 1st of each month for the chosen monthly payment plan regardless of holidays. If any credit card payments are returned there will be a $30.00 card processing error fee added to that months lesson charges after ONE invoice warning sent to the responsible party of a card processing error instance. This fee will be charged upon a second processing error.

______ My card will be charged for failure of a 2 week written notice of discontinuation or change in reoccurring lessons. Student is invited to finish out their final lessons after the 2 week written notice has been sent to Joy Music Studios to notify their pause or discontinuation of lessons. There may be fees added to the student to complete payment depending how many of their classes have been completed before their cancellation in the school year, if they still have more lessons to complete that have been paid for, the student will be invited to complete these lessons before concluding their lessons. Otherwise they can conclude but will not be given a refund for any previously paid for lessons. This is in the event that a student chooses to end their monthly lesson payment plan earlier than the end of the term they signed up for.

I agree to take one virtual class upon beginning music classes with Joy Music Studios within the first 24 months of classes. (or otherwise scheduled at instructors request) in order to experience a virtual class. *This field is required due to covid and other events that may arise and result in a virtual class opportunity.*
