Adult Photo Consent Form

I   First Name   Last Name   hereby grant permission to Service For a Cause Foundation Inc. (SFAC) to use photographs and/or video recordings taken of me at any SFAC function/activity for the purpose of promoting the initiatives of SFAC. I understand that these photographs and/or video recordings may be used on SFAC's website and social media platforms for non-profit and promotional purposes only.

I agree that these photographs and/or video recordings may be used for promotional materials, including but not limited to brochures, flyers, newsletters, and other promotional materials generated or used by SFAC.

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown.

I understand that my name and identity may be revealed in descriptive text or commentary in connection with the images. I release SFAC from any and all claims and demands arising out of or in connection with the use of the photographs and/or video recordings, including any and all claims for libel or invasion of privacy.

I am of full age and have the right to contract in my own name. I have read this release and fully understand the contents, meaning, and impact of this release.

This release shall be binding upon me and my heirs, legal representatives, and assigns.

(SFAC) to use photographs and/or video recordings taken of my child at any SFAC function/activity for the purpose of promoting the initiatives of SFAC. I understand that these photographs and/or video recordings may be used on SFAC's website and social media platforms for non-profit and promotional purposes only.

I agree that these photographs and/or video recordings may be used for promotional materials, including but not limited to brochures, flyers, newsletters, and other promotional materials generated or used by SFAC.
