▪️Being specific about size and body placement means a more accurate price and time estimate.

💞 Even for matching tattoos, each person must fill out their own request form, since all appointments are booked individually.

📧 I reply to submitted forms via email. Please make sure to provide an email address that you have access to and that is entered correctly.

By signing I understand that all deposits left with The Fox & Fable are nonrefundable! Deposits are able to be rolled over if you give at least 72 hours notice to give us ample time to fill your spot! If not I agree to forfeit my deposit amount. By signing I also understand that appointment dates may change on the artist ends at any time due to last-minute doctors appointments/travel dates and that my deposit will roll over onto the reschedule date discussed. By signing I understand that the deposit amount is the artists drawing fee and is non refundable. I also understand that no changes can be made to the design after submitting this form. By signing I understand that visitors are not allowed into the studio at all and under no circumstances. You will need to come to your appointment alone. If you have diabetes or any skin conditions please be sure to bring in a doctors note stating that it is OK for you to be able to get tattooed with your condition. We do not allow the use of numbing cream at our studio unless purchased from us. You will need to schedule a patch test at least a week before your scheduled appointment as well to see if you have a skin reaction.
