There are contraindications to a massage: metastasis cancer, malignant and uncontrolled high blood pressure, distressed people like in pre-eclampsia or pre-epileptic, history of stroke or aneurysms that isn't under control or regulated, loss of consciousness, trauma that needs immediate medical treatment, and other conditions. But generally, massage is good for most people. The benefits of massage therapy are amazing for putting a person into the rest and digest state of autonomic nervous system facilitation of healing. Many patients of any doctor are in the stressful state made hyper by anxiety, fear, and being forced to wait to be seen, analyzed, and assessed a diagnosis before given a management plan and treatment. It has been a part of the scientific basis of how chiropractic works, that the pain we feel from nociceptors come from immobilization of those joints around the body and proprioceptors in the joint capsule and under the skin that travel to the peripheral nerve root and synapse at the disc level of the spinal cord where they are then sent to the brain after crossing the cord to the anterior horn of gray matter (light touch, touch, pain, temperature) of the spinal cord or the dorsal columns of the spinal cord (proprioception, vibration of joint, 2 point prick sensation to skin, pressure, and itch) white matter depending on type of discomfort, and up to the sensory cortex of the brain to then travel to the cortex for interpretation. All pain is interpretated at the cortex level, all sensory except smell is interpreted at the cortex of the brain. The side of the brain is the opposite side of the body as the pain source, and when it sends out a signal from the cortex, its sent to the motor cortex for that region of body down to the side of the nerve root. At the nerve root the body is sent to release the pro-inflammatories from the cord for pain to lessen function and peripherally to the site of injury for substance P, interleukins, histamine, bradykinens, etc. and also slight pain reduction by use of the pain inhibiting chemicals at the dorsal horn of gray matter in the spinal cord in lemniscus 2 or lamina 2, where substantia gelatinosa or a deriviative of seratonin that is an opioid like substance of enkephalons that block the pain at the synapse point to inhibit more pain and allow healing locally without other detrimental side effects of pain from thought processes. Thoughts, traumas, and toxis are the mainstay of the original chiropractor and founder of chiropractic, Daniel David Palmer or DD Palmer. That subluxations or injury to movement, function, alignment that cause pain throughout the body are from negative thoughts, recent or old traumas to the body or mind, and toxins from chemicals and other contaminants exposed to that could be local inflammation or outside persisting factors.
You can see how massage therapy fits in, it releases the substantia gelatinosa pain relieving effect of seratonin and opioid like substances that inhibit pain. But also with the travel of pain, there is the other nerve fibers that can out race the pain signals on chronic pain fibers by their local site of sensation from pressure, temperature, or mechanoreception. Pain that is regular normal pain related to a recent injury that is healing and not fully healed versus old and chronic pain due to perpetuating factors, is made to travel on slower fibers called the thin old unmyelinated C fibers. Other pain or discomfort from nociceptors of irritants like chemical or itch travel on faster and slightly wider nerve fibers that are minimally myelinated on nerve fibers called the A-delta fibers that outrace the D fibers and can be a source of pain relief from chronic pain to more acute irritating pain. Then there is mechanoreceptors that are the same nerve fibers that reflexes occur and are due to mechanical response to injury or preventing injury that occurs directly in the spinal cord at the nerve root level without synapsing to travel up the spinal cord for interpretation. These mechanoreceptors travel on the nerve fibers that are the largest in diameter, the fastest in speed, and the most myelinated. They are the A-alpha and A-gamma nerve fibers for the muscle spindles of the muscle belly to prevent overstretch of muscles like hamstrings to avoid a tear of the fibers, or the golgi tendon organ (GTO) of the muscle tendon in the tendon that reacts to contraction of the muscle that could rip the tendon off the bone if not stopped immediately.
So, this is where physiotherapy, stretch, sports massage, deep pressure massage, therapeutic relaxing massage with less pressure, proprioceptive neuromuscular fascilitation occurs. It bypasses the pain receptors locally and can inhibit pain enough to allow local healing to patch and repair the site of injury triggering cortical pain or corticospinal tract interference that releases pain promoting biochemical agents of bradykinens, cytokines, substace P, interleukins, etc. and instead work on the mechanoreceptor tract of nerve fibers to inhibit pain and release enkephalons locally as well as opioid like factors of substantia gelatinosa called 5HTP or seratonin derivative product. This promotes relaxation to have less pain and activates the restful, digestive, relaxed, euphoric like, healing state. This should be done regularly to push out toxins that are pro-inflammatories that can erode tissues fo the body and impact proper circulation to soft tissues throughout the body.
The healing process from injury is always a state of inflammation, repair, and realignment of tissues to form stable scar tissue that might not be fully functional and has to be broken down to realign when it creates scar tissue that limits function or causes compression to other adjacent soft tissue and vascular or nervous tissues. The use of scraping, cupping, myofascial release technique, trigger point therapy, and cross fiber friction massage can help to realign old injured scar tissue to promote better function and more mobility of underlying tissue to prevent pain from additional compression on a nerve root or vascular structure it is impacting.
All soft tissue generally heals within 6 weeks more or less depending on the health condition of all systems of the person. All bones heal in 6 weeks generally when broken and realigned and there is no scar tissue on bone so it won't show on xray unless it is due to osseous overlay llike ankylosing spondylitis or osteoporosis of a joint or degenerative disc disease. And also in cases of lytic lesions destroying the bone like blood disorders of multiple myeloma, sickle cell anemia, and hodgkin's disease.
People with diabetes mellitis could have neuropathic pain and even lytic bone activity that can eat away at the bones in and around joints. Massage to the soft tissue areas of someone with diabetes mellitus type 1 or 2 would benefit from massage therapy, but for chiropractic treatment they are referred out for treatment by their health care specialist and also to avoid applying heating to the patient. It is contraindicated to apply a heat pad or any type of infrared heating to a person with diabetes if they have neurpathic pain they might not be able to feel the heat and it can burn through their skin in rare cases. There is a high incidence of diabetes in the USA that is attributed to the processed foods and lack of exercise daily as well as being sedentary. Many people will have diabetes and they are ok to massage.
Thyroid disorders will have sensitivities to temperature and if hyperthyroid will have an intolerance to heat and want the room cold, if a hypothyroid condition that is more common then they will be cold and want the heat up more. These changes can be made with massage and part of the sensory for client needs in a massage to have the environment be comfortable to promote an autonomic nervous system parasympathetic rest and digest response to healing. It is important to make sure the room and table are not too cold or too hot for each client and make them as comfortable as possible.
Many health conditions can be healed or managed appropriately, all people need to look at their whole health that includes their sleep, stress, daily movement, avoidance of long periods of being sedentary, exercise regularly, stretching throughout the day when sedentary, and managing pain and discomfort regularly like through massage therapy and other conservative management therapies like chiropractic, acupunture, herbal supplements, and also gut maintenance with probiotics and symbiotics to keep the gut flora healthy especially after use of antibiotics to kill pathogens. The gut flora is where we get are vitamin D synthesis and vitamin K as well as calcium for strong bones and muscle function, it also regulates the parathyroid hormone and electrolytes in the body for blood pressure regulation, kidney function, and repair of cell damage. It is important to keep all systems of the body active and working properly to live a pain free life and lower chances of illness and to speed of recovery after injury. The whole person is involved in any injury or illness to the body, and that is what you can help manage with massage therapy, is your holistic health.
