In order to setup a wholesale account, these are the items that we will need:

• Completed Wholesale Application
• If you are a LAc, DC, or MD, or qualifying Professional Healthcare Provider, you MUST submit a current healthcare license.

If you are a retail brick & mortar health products store or a health clinic with related online store, international online store or are a
practitioner of a wellness modality, you MUST submit the following:

• Copy of business license if one is required in your area
• Copy of seller’s permit if one is required in your state
• Copy of your healthcare license, certification, or training

All items above can be faxed to: (310) 919-1134, Attn: Wholesale Department, or emailed to wholesale@dragonherbs.com.

After we receive the above items, due to a heavy backlog of wholesale requests, please allow 2-3 days currently for us to review your application. Upon approval, you will receive an email confirmation with your account number.

Please keep these Wholesale guidelines in mind*:

• You must order 4 or more of each SKU to receive wholesale pricing. Quantities ordered of fewer than the required 4-unit minimum will be charged at full retail price. Some items also have higher minimum order quantities, which are specified in the price lists. Please refer to the latest updated price list for pricing and minimum requirements.

• LAc’s are exempt from this minimum requirement rule, except for Item #699 Heaven Mountain Goji Berries, which have a minimum order requirement of four (4).

• We offer Free UPS ground shipping based on orders of actual in-stock ordered items that total $150 or more after wholesale discount is applied. (When ordering less than $500 in product, shipping is determined by weight of package & distance to shipping destination.)

• There is currently no online wholesale ordering. However, agents are available by phone 7 days a week or you may email an order to wholesale@dragonherbs.com.

• Please note that your account will be periodically monitored to assess if your account is being used strictly for personal use. The intent of our wholesale program is for you to introduce our products to your clients and to promote the concept of Glowing Health, a goal that Ron Teeguarden looks upon as a sacred duty. An account created simply for the goal of a discount defeats this purpose and will be subject to review.

• We have implemented a MAPP or “Minimum Advertised Price Policy” (see page 2); please feel free to call us to learn more about how this benefits you and your business.

• Once approved, you MUST always submit your current healthcare license, business license, or seller’s permit information, if your license/permit is at or close to expiration. Otherwise, your wholesale privileges will be revoked until updated
information is received.

• In the future, we may differentiate between a Wholesale Account and a Practitioner Account which will be determined by your training and scope of practice and may affect what products are available to you at a wholesale discount.

• We have a 60-day return policy on wholesale accounts. All claims of shortage, missing or damaged products must be reported within 60 days of purchase to receive any refunds, replacements, or other adjustments.

Dragon Herbs Minimum Advertised Price Policy (MAPP)

To protect its reputation for quality products, Dragon Herbs (“DH”) has established a Minimum Advertised Price Policy (MAPP). All DH authorized resellers must agree to terms and conditions of the MAPP by signing below and faxing or emailing back to DH.

The DH MAPP states:

DH MAPP applies to all products sold by DH. DH reserves the right to stop supplying products to any dealer, wholesaler, Affiliate, or other accounts whose advertising of product (or to any distributor who sells to a dealer whose advertising of
the product) contains a price lower than the DH Retail MAPP Price, set forth in current DH Price List.

This policy applies to all forms of dealer advertising, including but not limited to mailings, catalogs, displays at exhibitions, conferences, seminars, shows, and any and all other forms of advertising media, including without limitation, the internet, any and all search engines, and/or electronic network, known or unknown.

Any price information relating to DH products on an Internet website which can be accessed directly through any transfer protocol (http) isadvertising for the purposes of this policy.

• Resellers may set the resale price of any product; however, advertised prices must be at or above MAPP for all DH products.

• DH MAPP applies to advertising placements, including but not limited to print ads (inserts, magazines, newspapers, catalogs, mail order
catalogs, etc.), broadcast (radio and TV), direct mail, faxes, internet placement (banner ads, broadcast emails, destination pages, third-party sites) or reseller’s own website, flyers, posters, coupons, and/or all other forms of advertisement.

• Resellers are not required to list prices in advertising; if a price is listed, it must be at or above MAPP.

• Resellers may omit pricing entirely from ads and/or include statements such as “Call for Price” or “Call for Quote.”

• Resellers are responsible for ensuring pricing is at or above MAPP on internet search engines.

• Resellers may never permanently discount DH products by more than 5%.

• Pricing listed on an internet site is considered “advertised price” and must adhere to DH MAPP in the “virtual shopping cart,” through check- out and final sale.

• Wholesalers/Affiliates may advertise DH products at price “mark-downs” if advertised price is at or above MAPP.

• Occasionally, DH may discontinue or engage in promotions regarding products. In such events, DH reserves the right to modify or suspend the MAPP for affected products by notifying all resellers of changes. DH further reserves the right to adjust MAPP for any products at its sole discretion, upon advance written notice to resellers.

• If a DH authorized product ‘Special’ promotion is underway with a DH product, resellers may advertise a price lower than MAPP, provided the reseller notes (below the price and on the same page) that the price reduction has been deducted from the advertised price and the amount of the Special. Advertising a Special outside of the authorized Special timeframe is not allowed and is considered a violation of
the MAPP.

• Free shipping and/or handling does not violate the MAPP.

• Intentional and/or repeated failure to abide by this policy will result in termination of reseller’s agreement. DH will not do business with resellers who degrade the image of DH and its products, nor provide prior notice or issue warnings before taking any action under this policy.

• DH MAPP terms are confidential and should not be disclosed to other parties. This MAPP has been established to ensure DH reputation as a leader in the health and wellness industry and protect the reputation of its name & products.

• This policy concerns advertised prices, not sales prices of any item; it applies equally to all DH distributors/dealers

Online Sales and Web Usage Content Agreement for Photos and/or Copyrighted Material
• Dragon Herbs, with all rights to the photos of its products and images, grants Distributor the specific usage rights to display or print the photos for the promotion of DH products only. DH excludes all other usage rights.
• These rights may not be transferred to or purchased by any other individual or entity.
• Distributor agrees to develop/maintain a unique website that is in no way confusingly similar to www.dragonherbs.com.
• Distributor agrees not to sell DH products on any websites that Distributor does not own & operate (e.g. Amazon, EBAY)
• Distributor understands that Distributor is not the exclusive distributor, online or otherwise, of DH products.
• Distributor agrees not to use photos as generic photos for similar products that Distributor may market or distribute.
• Distributor agrees not to modify, alter, edit or change DH photos.
• Distributor agrees to only use DH photos on sites which Distributor directly owns, runs and maintains. Photos are not transferable for
use on other sites without DH consent.
• Distributor agrees to inform DH of any photo infringement, honor and respect DH wishes & protect DH materials.
• Either party may immediately terminate this agreement with written notice if the other party breaches material terms of agreement or ceases to conduct business in the normal course or becomes insolvent.

The Dragon Herbs MAPP and Online Sales and Web Usage Content Agreement was established January 1, 2012 and is effective when the wholesale application has been signed and the account has been approved

I warrant all the above information and attached documents to be true to the best of my knowledge and authorize its verification. Dragon Herbs reserves the right to change its pricing and policies without prior notice or rescind this offer. This program is valid only if approved by Dragon Herbs authorized staff and is not
retroactive for any purchases made prior to the approval date.

I will claim no monetary damages if my application is declined, or my
discount status is revoked or cancelled. I acknowledge that the terms of this program are for my personal use only and cannot be used by or for other individuals or businesses. I agree to uphold ethical usage and business
practices with Dragon Herbs products and services, as dictated by the FDA and other governing agencies. All usage of Dragon Herbs logos, photos, descriptions and any other proprietary information require prior written approval.

Upon approval, in order to maintain program status, all applicable licenses and permits must be kept current WITH proof provided to Dragon Herbs proactively, and account must meet all Wholesale Program terms and conditions set forth. I understand that I may be asked to show a government issued photo ID at the time of each transaction.
