Waiver and Release of Liability (CONCEPT SCHOOLS CYSP INTERNATIONAL TRIPS)
For Applicants under 18 years old
In consideration of being allowed to participate in the abovementioned activity or field trip, I, the undersigned:
● Allow my child to participate in the abovementioned activity or field trip.
● Pledge my cooperation in making her/him aware of the precautions, as necessary, and in urging that she/he observe the precautions and any other instructions during the activity/field trip.
● Authorize any medical treatment in case of an emergency and agree that I am responsible for the cost of such treatment.
● Agree to release, hold harmless, and indemnify Concept Schools and its agents, representatives, and employees from all claims, damages, or other liabilities for injuries to my son/daughter which are not the result of gross negligence, intentional neglect, or willful conduct by the school, or its agents, representatives, or employees.
● Authorize Concept Schools to use my child’s still or motion pictures in any school publication, website, or video cassette on this trip.
