Why are we asking you to fill out this form?
This questionnaire is a useful tool to track changes in the day-to-day behaviors that have been shown to significantly affect our mental health. The results are used to help us measure and monitor changes over time and also help us to evaluate changes we can make in how we are supporting you.
What is being measured?
This assessment is designed to take a "snapshot" of the behaviors that contribute to your wellbeing over the past week. By limiting your responses to your behaviors for this past week you will be getting an accurate assessment of what you were actually doing and how you were actually feeling during this specific time period.
This past week (7 days) was not a "typical" week for me. Will the results still be useful?
However "typical" or "atypical" this past week was for you, this self-assessment will give you an accurate measurement of your behaviors for this time period. Since our life circumstances are constantly changing, our behaviors and subjective sense of wellbeing, are constantly changing as well. We can only begin to understand our wellbeing if we take accurate measurements of our actual behaviors over time.
How often should I take the self-assessment?
These questionnaires will be provided at regular intervals depending on your treatment plan.

This questionnaire typically takes under 15 minutes.
How will I get my results?
Your results will be displayed on screen after submission. We will also email you the results, so that you can discuss them in your next integration call. If you're an existing patient, but don't have an integration call scheduled, or you are not an existing patient and would like to discuss these results, please don’t hesitate to call or text us at (415) 675-1335.
What will you do with my results?
Your results and all information submitted are private and for your use only. We do NOT share, sell or transmit your results to anyone. We do NOT share, sell or transmit your results to anyone unless you give us explicit permission and request us to do so (for example, your psychiatrist, primary care physician or other providers).
