Agreement
As the parent / legal guardian of the child(ren) listed above, I hereby agree to have my child(ren) participate in this year's Arabic School Program at St. Mary Church. By agreeing to this, I allow the child(ren) listed above to electronically communicate with teachers and other students.
Food Allergies
Parents are responisble to disclose the level of need for their child's allergy to food. St. Mary can only make parents and children aware of food allergies during snack times; however, St. Mary does not assure parents that food items served are free from those allergens. Parents of children with sevre allergies should provide appropriate and safe food for their children. Parents may request special seating for their child during snack time to limit contact with foods that cause a severe reaction. VCS will take place during the Dormition Fast so we will only serve lenten snacks so that we will participate in the fast.
Photo Release Waiver
We will be taking photos during St. Mary Sunday School which will include photos of your child. By signing below, you allow St. Mary's to publish these pictures on the church's social media.
Waiver of Responsibility
As the parent / legal guardian of the child(ren) listed above, I hereby agree to have my child(ren) participate in the St. Mary Arabic School sponsored by St. Mary Orthodox Church, Palos Heights, IL, its subsidiaries, sub-organizations, committees, and/or their agents, employees and/or volunteers (hereinafter collectively known as the “church”). Further, I, the parent and/or legal guardian, expressly waive any right or claim I or my child(ren) may have against the Church, resulting from any injury arising out of or participation in said activity, and state that I allow my child(ren) to undertake this activity of my own free will and assume any and all risks arising out of said activity.
I hereby grant the Church the right to render or have rendered emergency medical care to my child(ren) while said child is in their care, if in the sole discretion of the Church, said emergency medical care is deemed necessary or prudent.
I hereby agree to indemnify and hold harmless the Church from all demands, claims, actions or causes of action, losses, damages, suits, judgements, cost, attorney fees and expenses incurred by reason of any claims, obligations, or liabilities against my child(ren) or me arising out of said activity.
