Furever Friends is a small dog rescue group located in Enid, OK.

Most generally our dogs are under 30 pounds. Most of our dogs are kept at our rescue facility where they  are free to roam and not kept in kennels or crates. 

Occasionally we are able to take in larger dogs if a foster home is available.  We rescue dogs from our local shelter, surrounding shelters, strays and owner surrenders.  

Our group is made up from volunteers.  We ask that you be serious about adopting before submitting an application as not to waste anyone's valuable time.  Since we are volunteers and have other jobs it may take between 1-7 days to process an application.  The application process will go faster and smoothly if all questions are answered on the form. 

We receive no grant, state or federal funding.  We operate totally on the kindness and support of those who donate. 

 

 

 

This questionnaire is designed to help FurEver Friends Rescue and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. FurEver Friends Rescue reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!

FURever Friends will not knowingly adopt any animal:

to individuals under 21 years of age
to a person with a history of animal abuse or neglect.
to homes where current pets are not neutered or spayed.
to be used for breeding/adopted to breeders (notice: all pets are spayed or neutered prior to adoption per state law).
for the purposes of vivisection, experimentation or blood sports.
to be slaughtered for food or to breed animals for slaughter.
to be used as an attack guard dog.
to be turned over to a person who has been refused an adoption.
to be given to another person who is unaware of the adoption to a residence where pets are not allowed.

​In consideration FURever Friends has agreed to allow me to adopt/foster the above described animal as my own family pet, subject to the terms and provisions of this agreement, I, the adopting party, agree as follows.
1. The adoption fee is NON-REFUNDABLE. If a fostering parent decides to keep the animal all funds used to either adopt or provide medical care will be refunded to FURever Friends . Adoption fee(s): Must be paid in full the day of adoption. Returned checks will be subject to a return check fee of $35.00 plus any court costs.
2. I agree to assume the full financial responsibility for the animal I have adopted/fostering.
3. If the animal or other animals in your home comes down with any illness, FURever Friends cannot assume the expense of veterinary care for the animal or any other owned animals if adopted.
4. I will provide the animal with all necessary inoculations at intervals directed by a veterinarian
5. I agree to keep the pet on heartworm preventative for all 12 months of the year
6. I agree to have current ID tags attached to a properly fitted collar on the animals at all times, indoor/outdoors.
7. I agree to license this animal within thirty days of ownership, if over 4 months of age, if required by your city/county ordinance.
8. I will never allow this animal to ride in the back of any open pickup truck. I will never leave the animal in a parked car alone.
9. I understand that the animal was placed to live in a private residence only, not for resale, breeding, fighting and/or bartering
10. I agree to properly restrain the animal on a leash or other tether when it's not on my property.
11. I agree to maintain a securely fenced yard on my property which includes shelter from all elements. Furever Friends will individually look at each adoption application; fences are not required in all circumstances.
12. I authorize volunteers from FURever Friends the right to visit and check on the welfare of the animal adopted iasked.
13. Rehoming: If for any reason you have to "rehome" an animal that you adopted from FURever Friends within 12 months from the date of adoption you must notify FURever Friends that you can no longer keep the animal. At the time of notification if FURever Friends has availability to take the animal back they must be given the first opportunity to do so. If they do not have availability to take the animal back they must be notified of who the animal was rehomed to so they can update their records. The animal must not ever be taken to a shelter of any kind, if the animal is taken to a shelter you may face a fine of $500.00.
14. Adopting party/Foster party agrees to provide adequate attention, food, love, water, shelter and medical attention.
15. I understand that no warranties, either expressed or implied are made regarding the following: Breed of animal, Health or medical history of the animal, Personality or disposition of the animal.
16. I understand that failure on my part to fulfill any of the requirements of this agreement or a violation of any of its terms constitutes a material breach. In the event of such a breach, Administrators' of FURever Friends at my opinion but without limitation may reclaim the animal from me or any member of my family who may be in possession of the animal. In the event that of such reclamation or as a result thereof.
17. I agree that should any legal action be required to enforce this agreement or any of its provisions, I will pay such cost as may be awarded by the court in addition to all attorney fees
18. Release of liability: The said animal __approximately _of age has been checked by a licensed vet and given the appropriate vet care based on its age and findings. The adopter would like to go ahead and take the animal into their custody releasing FURever Friends from any future liability of any diseases/parasites that may affect this said animal.
19. Applies to FOSTER TO ADOPT ONLY (Animals not fully vetted). I have agreed to Foster to Adopt the animal listed on page 1. Permanent ownership of this animal is not final until all appropriate vet care has been done. Including spay/neutering of this animal by the age of 5 months. Spay/Neutering of this animal will be done at the vet chosen by the administrators of FURever Friends Fund and scheduled by them. It’s the responsibility of the foster to bring the animal to ALL vet appointments including the spay/neuter appointment on the day scheduled. In the event the foster does not comply with this requirements the animal will be returned to FURever Friends and will not be entitled to a refund and must also pay a $1500.00 fine for failure to comply with this agreement. FURever Friends also maintains the right to be able to contact the veterinarian and receive records / reports regarding this animal. It is the responsibility of the foster to provide with all necessary inoculations at intervals directed by a veterinarian.(Vaccinations, rabies, spay/neuter and any other vet related needs.) The adoption fee covers vaccinations, spay/neuter, microchip, other routine veterinarian care.

I have read and understand the requirements in the adoption/contract and agree to foster to adopt this animal until it is fully vetted. Permanent ownership will take place at the time said animal is fully vetted including spay/neuter.
20. SPAY/NEUTERING
By my checking the box below, I agree to have this animal spayed/neutered by and no later than (date:) ____________, and I understand that this spay/neuter agreement is an agreement that this animal will not produce a litter of puppies/kittens either as purposely bred or by accidental breeding. I also agree that should animal not be altered that I will return said animal back to FURever Friends with no refund and must also pay a $1500.00 fine for failure to comply with this agreement. Permanent ownership of this animal is contingent upon your compliance of the spay/neuter.
The said animal may need additional vet care as indicated on the medical record sheet. The adoptee must assure that the animal gets the required vet care as described. Failure to do so will result in immediate termination of this contract and the said animal must be returned to FURever Friends. The adoption fee only covers the vet care already provided to the animal unless otherwise stated. . I agree that the required vet care still needed as described on the last page of this agreement will be done on time at the expense of the adopter unless otherwise stated. Failure to provide the animal with appropriate vaccinations on a timely manner will not be covered under the adoption fee. Vaccinations must be done 3 weeks apart to keep your new pet healthy.
DISCLAIMER
FURever Friends, its volunteers, coordinators, and representatives; and any other person, firm or corporation involved with the efforts or business of this group, shall not be held liable or responsible for any and all claims, damages, costs, expenses, loss of services, actions and causes of action incurred as a result of the provision or restriction of information, services, contacts, adoptions, fostering, veterinary care or by the actions of any and all dogs and/or cats transferred or referenced herein.
It should be understood that applications for dog and cat adoptions are subject to acceptance based on a review process that may require collecting information from references. Decisions on placing dogs and cats in adoptive and foster homes are an 'art,' not a 'science' -- there might be times when applications will be denied for various reason(s). FURever Friends reserves the right to make all decisions regarding placement or final disposition of any rescued dog or cat in its care whether in, or being placed in, a foster or adoptive home or other facility. There is absolutely no guarantee made or implied that any person, or persons, requesting to adopt a dog or cat being fostered through, FURever Friends or posted on FURever Friends facebook page, will be granted an adoption!
