This form is for participants joining in regularly scheduled children's ministry programs including Friday night Kids.

• Activities include: worship music, arts and crafts, games, Bible story tellers, excursions to Mimico Memorial Park

Dear Parent:

As part of our Children's Ministry programming we require your permission for your child to participate. We have provided you the details of the activity and request that you complete and sign this permission form. Our Children's Ministry staff/volunteers are screened through our church's child protection policy. We expect all children to follow the direction of our staff/volunteers; any ongoing behaviours will be addressed with parents.

The safety of your child is our primary concern. Precautions will be taken for their well-being and protection.

I/we hereby consent to the participation of my/our child in this supervised activity including travel to and from the locations listed above.

While every precaution is taken for the safety and good health, some activities carry with them the inherent risk of personal injury beyond the risks associated with this event. I/we understand and accept these risks and agree that by allowing my child to participate in those activities, he/she may be taking part in a recreational activity that presents the potential for personal injury.

I/we, the parents or guardians named below, authorize the Pastor or designate of Mimico Baptist Church (MBC) to sign a consent for medical treatment and to authorize any physician, dentist or hospital to provide medical assessment, treatment or procedures for the participant named above.

I/we, named below, undertake and agree to indemnify and hold blameless Mimico Baptist Church, its Volunteers, Staff, and board of Directors and Officers from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Mimico Baptist Church, as well as of any medical treatment authorized by the supervising individuals representing MBC.

I agree to ensure my child understands the expectations including but not limited to following the instructions of MBC staff. Any on-going behaviours may result in the removal from the program.

I grant MBC permission to contact me about any activities initiated or endorsed by MBC.

I grant permission for the reasonable use of pictures or video of my child in any MBC publications (in print, online or other media yet to be invented).

I have read, understood and agree with above.

 

我/我们在此同意让我/我们的孩子参加此受监督的活动，包括往返上述地点的行程。
