To help us find and address the issue quickly and efficiently, please provide us with as much information as possible. You can also upload screenshots showing the issue that needs to be resolved with the file upload field.

We appreciate your help and feedback with continuous improvements to our website and features!

Please upload any helpful screenshots showing the issue you are reporting. If you are submitting screenshots for several issues, please save the screenshots with a file name that clearly identifies it with the specific issue you are reporting. Also provide a detailed explanation and information that will help us identify which screenshot pertains to which issue. 

 

Example: Jennifer finds two issues on the provider search. One issue is showing a glitch with the Provider Search tool while the other is showing a mistake in copy on the Flu page.

1. Jennifer grabs a screenshot of the glitch. She saves the screenshot and names the file provider-search-glitch.png and notes the file name in the field Please Describe the Issue or the Error.
   
   
2. Jennifer also grabs a screenshot of the copy error on the Flu page. She saves a screenshot and names the file flu-page-copy-error.png and notes the file name, along with the correct information, or with the name of the person who would be able to provide this information in the Please Describe the Issue or the Error field.
