Please fill out and submit this form (NO LATER THAN NOVEMBER 1, 2026) to participate at Wahiawā's Day at Lake Wilson. Mahalo for participating and supporting our community!

Please read the attached contract below. Submission of this form will act as acceptance of the contract. Vendors are not required to have a Certificate of Insurance to participate. Vendors selling are responsible to have their GE license on hand. Food vendors must display their Dept. of Health certificates at the event.

Vendor information will be sent to you a week before the event with parking, setup, and breakdown information.

Day at Lake Wilson is on Saturday, November 7, 2026 from 9 AM - 2 PM.

Vendors must supply their own 10x10 popup tent, table and chairs.

I have read the contract for the 2026 Wahiawā Day at Lake Wilson and will abide by its terms. I will release Wahiawā Health and Wellness Association, Hawaii DLNR, and all organizations associated with the festival from any and all liability for personal injury or loss or damage to personal property arising out of my participation inthe Event.

I understand that the signing, and submission of this form and payment will act as acceptance of the contract.
