In order to participate in Hunter After Dark After Hours events at the Chattanooga Public Library, teen attendees between the ages of 12-18 must have this form completed by a parent or guardian.

I will be at the Main Library no later than 8:00 pm to pickup my teen, or my teen will be driving to and from the library on their own and will leave the Main Library by 8:00 pm.
