I am a client and understand and agree to the following:
TERMS OF SERVICE
1. This contract applies to all veterinary services provided by Massie Equine Veterinary Clinic & Healing Center, Inc., to any and all horse(s), pets and livestock, on owner's behalf, whether or not the animals are listed.
2. Payment is due at the time service is rendered.
3. Client must have a card on file. Client has provided Massie Veterinary Services with a valid credit card number and hereby authorizes Massie Equine Veterinary Clinic & Healing Center, Inc to charge the credit card for services rendered in accordance with the terms of this agreement. There is a 3% credit card services fee added for card transactions.
4. Clients may elect for automatic payments. Automatic payments shall enable Massie Veterinary Services to charge the current balance due at time of service to the credit card stored via PCI Compliance. A receipt will be sent electronically with payment confirmation.
5. If payment for services rendered is not received in full within 30 days of service, Client hereby authorizes Massie Veterinary Services to apply the charges to their credit card on file.
6. Client's may pay by personal or business check, cash or card. Client is responsible for all bank charges incurred by Massie Veterinary Services if a check is returned, in addition to a $25 (twenty-five dollar) service fee.
7. Unpaid balances are subject to a 2% monthly interest charge (24% per annum) and a $15.00 (fifteen dollar) per month accounting charge.
8. Deposits equal to farm call or exam are required to hold your appointment. No exceptions. Deposits are not refunded except in the case of euthanasia, if your animal passes prior to your appt. If you cancel your appointment, your deposit can be used toward future services.
9. When paying via credit card, a copy of your bill and credit card receipt will be mailed or emailed to you.
10. We encourage our clients to ask for an estimate prior to their appointment. It is the client’s responsibility to request and understand the costs of services before they are provided
11. Unfortunately, we are unable to provide services to clients with outstanding balances of more than 30 days.
12. Emergency services may be provided upon approval. Finance charges will be applied to past due balances (over 30 days from date of service).
We do not bill insurance companies but will fill out paperwork for you to be reimbursed. If your animal is insured, plan to pay for veterinary services directly.
13. If we are unable to confirm your appointment within 48 hours of the scheduled visit we will open our schedule up for other clients. If a scheduled visit is canceled, within 24 business hours, a cancellation fee of $50 will be assessed.
14. For patients that will be boarded at the clinic, one half of the estimated cost of the treatment, or the “base fee” for the desired service(s), is required at the time your animal is admitted. The remainder of the balance is due when your animal is discharged.
15. Outpatients that are not being kept at the clinic must have balances paid in full at the time service is rendered.
16. Unless otherwise requested, all invoices, medical reports and diagnostic images shall be provided electronically.
17. Massie Veterinary Services has the right to refuse service at any time. Massie Veterinary Services will not provide service to Client's with accounts that are past-due beyond 30 days.
18. Client shall immediately notify Massie Veterinary Services if he/she is unable to comply with the terms of this agreement. Massie Veterinary Services reserves the right to waive fees and modify payment deadlines on an individual, case-by-case basis at its sole discretion. Such an agreement shall be effective upon the execution of a separate, written agreement, signed by both parties.
19. Except as otherwise provided in this document, this agreement may be modified, superseded, or voided only upon the written and signed agreement of Massie Veterinary Services and Client. Further, the physical destruction or loss of this document shall not be construed as a modification or termination of the agreement contained herein.
20. Massie Veterinary Services stores credit card numbers in compliance with Payment Card Industry Data Security Standards (PCI DSS In accordance to regulations, credit card numbers are stored on a firewall and password protected computer, in a financial software program that is also protected by software and complex passwords. As such, Client's credit card number relating to this agreement shall be submitted on a separate form and deleted immediately after being entered into the secure software.
21. The rights and obligations of the Parties under this Agreement shall be governed by the internal substantive law of the State of Oregon applicable to contracts made and to be performed in that State, without giving effect to the principles of conflicts of laws. For the purpose of jurisdiction and venue for all purposes of this Agreement shall be Jackson County, California. If any legal action, arbitration or other proceeding is brought for the enforcement of the agreement, or arises out of an alleged dispute, breach, default or misrepresentation relating to any of the terms of the agreement, the prevailing party shall be entitled to recover reasonable attorneys fees and other costs in that action or proceeding in addition to any other relief to which it may be entitled.
22. Client must list any agents or representatives that may request and authorize veterinary care or order supplies on behalf of Client. Changes to veterinary agent or representative authorization must be submitted to Massie Veterinary Services in writing, by email to: reception@massievet.com and confirmed by Massie Veterinary Services (within 1 business day of receipt) in order for Massie Veterinary Services to avoid any unauthorized veterinary service charges. Massie Veterinary Services shall not be responsible for verifying current agent or representative authorization.
I hereby authorize Massie Equine Veterinary Clinic & Healing Center to provide routine and emergency care to my horse(s), pets and livestock, in my absence or at the request of the below listed people. I agree to accept all financial responsibility for services ordered by the below listed people on my horse(s)'s behalf:
