Accommodations Request for CVC Events
This form is for students with disabilities to request an accommodation when they attend a CVC event. If you simply want to RSVP for an event, please do not use this form. Whenever possible, submit this form at least two weeks before an event. The CVC will process your request and let you know if the accommodation can be provided.
Please note that this accommodation form is for attendees of CVC events ONLY. If you would like to request an accommodation for academic services, visit the "Learning and Accessibility Services" page on the Wheaton College website and fill out their Initial Accommodations Request Form.
