I, the parent/guardian of the participant, a minor, agree that the Participant and I will abide by the rules of Norberto Ochoa Orejel and Ochoa Football Academy. I understand that there is risk associated with playing soccer, all sports and field related activities. In consideration, for any participated activities with trainer Norberto Ochoa Orejel or associated personnel and volunteers of the Ochoa Football Academy, my signature indicates that I assume the risk of any injuries that my child may sustain while participating with the Ochoa Football Academy trainings/clinics (Norberto Ochoa Orejel).

I consent that Ochoa Football Academy (Norberto Ochoa Orejel) shall have the absolute right and permission to publish any and all photographic portraits, videos taken of the Participant in publications, online, social media platforms and in other communications related to the mission of Ochoa Football Academy without compensation or approval rights.

I, the undersigned, have carefully read and understood this agreement and all of its terms. I understand that this is a release of liability which will legally prevent me or any other person from filing suit or making any other legal claim for any injury. I nevertheless enter into this agreement freely and voluntarily and agree that it is binding upon me, my heirs, assigns and legal representatives.

By signing this waiver and release of liability form, I acknowledge that I HAVE READ AND FULLY UNDERSTAND AND AGREE TO ALL OF ITS TERMS AND CONDITIONS. I further state that I have executed this waiver and release of liability form voluntarily and with full knowledge of its significance.

PARENTS AND GUARDIANS (if participant is under 18 years of age)

If the participant is a minor, the parent/guardian agrees to assume all liability and to HOLD HARMLESS AND INDEMNIFY Norberto Ochoa Orejel and Ochoa Football Academy against any actions brought by the minor participant against Norberto Ochoa Orejel and Ochoa Football Academy. The parent(s)/guardian(s) also accepts responsibility for all medical expenses incurred by the participant.

Refund & Cancellation Policy:

No refunds within 5 days of the program start, regardless of when payment is made. No refunds after the program begins for any reason, including missed sessions or personal circumstances. No-shows will not receive a refund or credit. There will be no makeup sessions for missed group sessions, and missed classes cannot be rescheduled. If Ochoa Football Academy cancels a session, a makeup class or credit will be offered. By enrolling, you agree to these terms.
Weather Policy

I acknowledge that training will proceed as planned, regardless of rain, unless thunderstorms arise once the session has started. If thunderstorms occur during training, session will be cancelled.
