I, the undersigned, hereby acknowledge that I have read and understand the following:

1. Nature of Service: I understand that “permanent jewelry” is a type of jewelry that is applied using a welding process and is intended to be worn continuously. I am aware that there is a risk of burns, skin irritation, or allergic reactions associated with the welding process.

2. Consent to Service: I voluntarily consent to receive permanent jewelry services from PIPER-jade Located at 707 Texas Ave Suite 102D, College Station Texas 77840 or at 1500 Harvey Rd Suite 7020, College Station 77840. I understand that the service will be performed by trained professionals, but I am aware that there is a possibility of human error.

3. Assumption of Risk: I assume all risks associated with the permanent jewelry service, including but not limited to, burns, skin irritation, allergic reactions, infection, or other bodily harm.

Release of Liability: I hereby release PIPER-jade Located at 707 Texas Ave Suite 102D, College Station Texas 77840 or at 1500 Harvey Rd Suite 7020, College Station 77840 its owners, employees, and agents from any and all liability for any injuries, damages, or losses suffered by me as a result of the permanent jewelry service.

4. Minor Consent: If I am signing this waiver on behalf of a minor, I certify that I am the legal guardian of the minor and have the authority to give consent for the service. I understand that the minor may experience the same risks as an adult and I assume full responsibility for any injuries or damages sustained by the minor.

5. I fully understand that all SALES on Permanent Jewelry including any chains, charms and or engravings at PIPIER-jade is all final sale. I understand that due to the customization of this product that no refunds will be issued at anytime.

I have had the opportunity to ask questions about the permanent jewelry service and have received satisfactory answers. I understand that this waiver is a legally binding document.
