Registration Form
Windsor School admits students throughout the academic year subject to availability. Students wishing to attend Windsor must supply the following documentation:

Initial Inquiry:
• Completed Registration Form
• Complete Academic Records - previous 2 years minimum - for grade 1/year 2+ applicants
• Complete High School Transcript - for grade 10+ applicants
• Two Teacher / Personal Recommendations - for grade 1/year 2+ applicants, at least one recommender must be a teacher who has taught the candidate for a minimum of a year
• Birth Certificate or Valid Passport - color copy

Upon Enrollment:
• Medical and Release Forms
• Insurance Form
• Likeness Waiver/Media Release
• Personal Care Form
• Two Passport Style Photos
• Behavioral Health Profile

Lodgers will also need to complete Lodging and/or Student Visa Forms. Not all forms are required for all grades.
