AHC Academy School Bus & Consent Release Form

The Abu Huraira Center Academy in partnership with BAWA transport will be providing a school bus service throughout the school year! This form is mandatory for all participants
Morning Arrival Time: 8:30 AM at AHC Academy

Afternoon Departure Time: 3:30 PM from AHC Academy

(Home arrival will depend on Bus Timing. Schedules for Pickup & Drop off will be shared through WhatsApp Bus Groupchat)

Price (per child/month): $180 (fee will be charged 23rd of every month)

I, Name of Parent/Guardian* understand that by allowing BAWA Transportation for my child/children’s transportation ‘to and from’ Abu Huraira Center Academy at 270 Yorkland Blvd with their School Bus Services I release the BAWA Transportation Group, The Abu Huraira Center Academy, The Abu Huraira Center, their agents, employees and school bus services staff from and against any and all liability, loss, damages, and death claims to the maximum extent possible permissible by the law arising out of school transportation.

I hereby assume all of the risks of participating in any/all activities associated with the pick up and drop off of all students. This including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
