    Please note:

    • The maximum file size is 10 MB per file.
    • There is no limit on the number of files you can upload.
    • If your file exceeds 10 MB or if you encounter difficulties uploading files, please email your files to info@thepetoncologist.com with the patient name in the subject line.

  • Standard consultation reports are typically provide reports within 3 business days. Please select a date that is at least 3 business days from today.

    Selecting a date earlier than the required timeframe may result in your request being charged at a higher priority level.

    Urgent consultation reports are typically provide reports within 1-2 business days. Please select a date that is at least 1-2 business days from today.

    Selecting a date earlier than the required timeframe may result in your request being charged at a higher priority level.

    Priority (Same-Day) - A concise report may be issued within reporting time, with detailed report to follow in 1-3 business days.

    Priority reporting is not available after-hours, on weekends, or on public holidays.

  • Concise (5-10 pages)

    • Essential info, practical focus, not overwhelming—all info still retained for case management

    • Best for: GP, busy clinics, straightforward cases, time-constraints, quick read, already familiar with chemotherapy/cancer, can ask more questions later

    Comprehensive (10-15+ pages) - Additional $100

    • Detailed explanations, ± references, detailed drug handling, side effects, sourcing + all possibilities explained

    • Best for: Specialists, complex cases, education, new to cancer or chemotherapy, clients who need in-depth understanding or have lots of questions

    Custom (Specify below)

    • Concise + provide specific requests—all information still retained for case management

    • Best for: Specific areas of concern, mixed experience levels, particular client/clinic/vet needs

  • FOLLOW-UP FEE: $100
    This fee applies to each follow-up and includes addressing questions or concerns about the case or report, reviewing results, and follow-ups over 6 months. It also covers urgent follow-up requests and related phone calls.

    BRIEF FOLLOW-UP FEE: $50
    This fee applies to each brief follow-up and includes short, simple responses or brief correspondences. It also covers the time required to assess whether the chemotherapy protocol or treatment option selected is appropriate based on the email or updated results provided.

    We will respond to all follow-ups within one business day, typically within a few hours.

    For all follow-ups, please ensure you attach all relevant medical records, correspondences, and results from the last communication with us.

    While filling out this form for follow-ups is not required, it is helpful. Alternatively, you may email us directly with all attachments.
