I acknowledge that this payment will be non-refundable once my child's place has been confirmed with external providers.
If my child is unable to take part due for medical reasons (excluding pre-existing conditions), I will provide a Medical Certificate that clearly states that my child is unfit to travel. This document will be required to support any claim for part of the payment that might be recoverable through the school arranged travel insurance policy.
I have read and agree to the Code of Conduct linked above. I understand that my child or ward will be asked to sign a physical copy of the form before they take part in the trip.
