Cross-Country Permission Slip 

Dear Parents/ Guardians, we are excited to offer your child the opportunity to participate in the 2024/2025 cross-country season at Creation Village World School. This dynamic sport encourages teamwork, resilience, and a strong sense of personal accomplishment.

Before your child can join our cross-country team, we kindly ask for your permission and acknowledgment of the following details:

 I give permission for my child, {studentName} , to participate in the cross-country season at Creation Village World School. I understand and agree to the following terms: 

• I am aware that my child will be engaging in physical activities related to cross-country running during practice and meets. 
• I understand that practices will be held on Mondays, Wednesdays, and Thursdays from 7:45 am to 8:15 am. 
• I understand that my child will need appropriate athletic gear, including a lightweight shirt, shorts, running shoes, and a change of clothes (school uniform). 
• I understand that my child will change into their school uniform after practice, either in the auditorium bathroom or classroom bathroom, as specified. 
• I acknowledge the cross-country meet schedule and understand that my child will be participating in the meets as indicated on the schedule. 
• I understand that the school will take necessary safety precautions during practices and meetings, and I release Creation Village World School from any liability for injuries that may occur during these activities.

Practice Times

Please sign and return this permission slip to your child's homeroom teacher by August 23.
