Thank you so much for participating in our first ever Legacy Section of Studio Visit Magazine! We are looking forward to featuring your work.

The next step in the process is to submit your materials for print. Every category you fill out below the section "Information to be Published" will be prepared for print.

Please organize these materials before you begin. You will not be able to make changes once they are submitted. And, you cannot save your progress as you fill out the form and return. It must be done in one sitting, which we anticipate should take 10-15 minutes if you are prepared.
