A special class designed to help get your child excited about music!
ABOUT THE CLASS:

MuzikMakers Rock Band for ages 4-6: Monday 3:15pm

• Class is 45 minutes in length each week

• $25 non-refundable enrollment is due upon receipt of this form by our office*

• Tuition: $116/month per child
• Instruments are provided in class
• No “homework” required between class

*waived for currently enrolled Moscow Music Academy students.
I wish to enroll my child in the MuzikMakers Rock Band class. I understand that a non-refundable $25 enrollment per child ($40 per 2 in a family) is due upon submitting the pre-enrollment form. I understand one-month tuition of $116 for the first class is due at the time of enrollment (prior to attending the first class) and that my child's class placement will be forfeited and the $25 enrollment fee will NOT be refunded if the tuition balance due is not paid. I understand that subsequent monthly tuition payments are processed by auto-pay by the 21st of each month.

To discontinue participation and tuition charges in ensemble class(es), the academy office must be notified in writing before the 15th of your last month. Written notices received on the 16th day of the month or later will become effective for the following withdrawal deadline. Tuition is not pro-rated for students not finishing out their last month of class.
