Big Cat LLC - Bodywork and Movement Lessons

Waiver & Release of Liability

 

1. Assumption of Risk

I acknowledge that the movement lessons and bodywork activities offered by Big Cat LLC, including but not limited to bodywork, structural integration, stretching, propper body mechanics lessons including functional movement exercises, weight lifting, mobility work, and other physical activities (the "Activity"), involve inherent risks. These risks include, but are not limited to, physical or psychological injury, temporary or permanent disability, and death. 

I understand these risks are part of physical movement and bodywork and agree to take personal responsibility for my participation, including communicating any discomfort, pain, or limitations during sessions.

2. Consent to Bodywork and Movement Training

I understand that the bodywork and movement training provided by Big Cat LLC are based on a holistic approach, aiming to align and organize the body rather than solely alleviate symptoms. I acknowledge that results from such activities can vary from individual to individual, and no specific outcomes are guaranteed. I consent to the necessary physical touch required to assist in establishing balance and alignment in my body. I understand that I may request to stop the session at any time, for any reason, and my request will be immediately honored.

3. Scope of Practice

I understand that Big Cat LLC does not treat, prescribe for, or diagnose any illness, disease, or any other physical or mental disorder, injury, or condition. Nothing said or done during the Activity should be construed as such. I understand that my Structural Medicine Specialist at Big Cat LLC provides guidance on body mechanics, focusing on proper form, posture, and techniques during physical activities like weightlifting. I further understand that Big Cat LLC is not attempting to practice chiropractic, physical therapy, athletic training, or any profession requiring a license (other than massage therapy) under the laws of Washington State.

4. Release of Liability

In consideration of being allowed to participate in the Activity, I hereby release, waive, and discharge Big Cat LLC, its members, agents, employees, volunteers, and affiliates from any and all claims, demands, or causes of action arising out of or related to any injury, loss, or damage I may suffer as a result of participating in the Activity, including travel to and from the Activity. This release covers any claims arising from negligence on the part of Big Cat LLC or others.

5. I agree to indemnify and hold harmless Big Cat LLC from any claims, damages, or expenses, including attorney's fees, resulting from my participation in the Activity or any actions taken on my behalf.

6. Compliance with Washington State Law

I understand that this waiver is governed by and construed in accordance with the laws of the State of Washington. I agree that any legal action or proceeding related to this waiver shall be brought exclusively in the state or federal courts located in Washington State.

7. I acknowledge that it is my responsibility to consult with a physician before participating in the Activity to ensure that I am physically fit and able to engage in the training. I understand that Big Cat LLC does not provide medical care, and if I require medical treatment during the Activity, I am fully responsible for the costs.

8. Use of Photographs, Video, and Audio I understand that photographs, video recordings, and audio recordings may be used as part of the treatment process to monitor results, analyze conditions, plan subsequent sessions, and educate me as a client. I consent to the use of these aids during my treatment sessions, knowing that they are confidential and will not be used for any purpose other than my treatment unless I provide additional authorization.

9. Property Damage I accept full responsibility for any damage to equipment, facilities, or property caused by my actions or those of my family during my participation in the Activity.

10. Severability If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions will remain in full force and effect.

11. Acknowledgment I have read and fully understand this waiver and release of liability. By signing below, I agree to the terms and voluntarily give up the right to bring any legal action against Big Cat LLC for any injury, loss, or damage related to my participation in the Activity.

Parent/Guardian Waiver for Minors

If the participant is under 18, this section must be completed by a parent or guardian: I certify that I am the parent or legal guardian of the above-named minor and consent to their participation in the Activity under the terms of this waiver.
