Swim Lesson Cancellation Form
Please read this information carefully before proceeding:
- TAC group and private lessons must be cancelled at least 24 hours PRIOR TO THE START of the class.
- If cancellation is illness-related, the class can be cancelled without 24 hour’s notice, BUT a doctor’s note is required to reschedule. Doctor’s note must be submitted within one week of missed class.
- If the class is cancelled, the lesson pass or credit will be returned to Amilia.
- Lessons cancelled with LESS THAN 24 hours notification will be charged in FULL.

By signing below, I understand that:
- To reschedule without penalty, my cancellation request must be submitted at least 24 hours PRIOR TO the start time of the class.
- TAC will accept late cancellations for illness-related reasons ONLY.
- TAC will only accept illness-related cancellations that include a doctor’s note.
- TAC will not accept any cancellation requests, regardless of reason, if submitted more than 7 days after the missed class.
