Westside Community Center Facility Rental Request Form
Smoking of any kind is strictly prohibited on Westside Community Center property. Alcohol may only be permitted with prior approval. After your event, you are responsible for a thorough cleanup, which includes removing all materials used, cleaning surfaces, vacuuming, mopping, and taking out the garbage. Additionally, you must set up and reset any furniture needed for the event. Please ensure you have enough volunteers available to assist with these tasks.

For added convenience, WCC offers a setup, breakdown, and cleaning service for an additional fee. This hassle-free option ensures your event space is arranged to your specifications, reset afterward, and thoroughly cleaned. If you'd like to take advantage of this service, simply select the option on the next page of the contract.

I agree that I am responsible to ensure that the premises are at all times kept in a safe and clean condition; and that no damage to or destruction of property or the premises itself occurs.

I agree that I am entitled to use only the Premises described herein, including parking areas, and designated means of ingress to and egress from the Premises on such date and during such hours as stated on application. If additional time is used beyond the contracted hours, the renter will be responsible for any and all costs incurred and will be subtracted from the security deposit.

I agree that if alcoholic beverages will be SERVED at the EVENT then the RENTER must: Provide evidence of applicable Host Liquor Liability Insurance coverage in accordance with the requirements set out in the Insurance Rider attached. The Certificate of Insurance must indicate and the coverage must provide that the applicable insurance policies are written on a per occurrence basis and have been endorsed to name the Westside Community Center as an additional insured party. Such certificates must be presented with this Application and shall indicate that all coverage remain valid until the expiration of the RENTER's responsibilities.

The RENTER is responsible for taking all necessary precautions to ensure the safety of everyone on the SITE during the event. The RENTER must promptly notify the on-site staff in writing of any safety issues they notice or become aware of before or during the event. The RENTER is also responsible for any damage to WCC's property caused by or related to the event and must pay for repairs based on WCC's assessment. This payment is due within thirty days and may be deducted from the Security Deposit without the RENTER's permission.

The RENTER must indemnify, defend, and hold WESTSIDE COMMUNITY CENTER and its officers, agents, and employees harmless from any lawsuits, claims, or financial losses (including reasonable attorney's fees) related to the RENTER's use of the SITE, or any actions or neglect by the RENTER, its agents, officers, employees, volunteers, guests, or anyone else on the SITE with the RENTER's permission. This obligation continues even after the agreement ends.

The RENTER releases the WESTSIDE COMMUNITY CENTER from any liability or claims arising from the event, regardless of whether the issue was caused by vendors or others involved in the event. The RENTER is solely responsible for the event's conduct and agrees not to hold the WESTSIDE COMMUNITY CENTER liable for any related claims. WESTSIDE COMMUNITY CENTER is not responsible for any damage or loss of the RENTER's property or that of their guests, agents, or invitees.

Insurance Coverage: The RENTER shall maintain, at its own cost, insurance coverage applicable to its use of the SITE as stated herein. Such coverage shall be maintained in effect throughout the term of RENTER's use of the SITE until such time as the WESTSIDE COMMUNITY CENTER indicates IN WRITING that RENTER has fulfilled its responsibilities hereunder. A Certificate of Insurance must be presented by the RENTER evidencing the coverage set out in the attached Insurance Rider. Insurance may be purchased through EVENTSURED at www.eventsured.com.

WESTSIDE COMMUNITY CENTER hereby agrees to rent the facility and all rooms referenced above with the understanding that the RENTER has agreed to abide by and fulfill the conditions set forth herein and in the documents attached, which documents are incorporated into this Facility Rental Agreement by reference.
