The intent of this form is to capture player availability for the fall, allowing us to finalize our plans. For more information, please refer to this document.

Please provide the best contact information in case we have any questions about your submission. Also, please note that we’ll be using TeamSnap for both the Fall and Summer seasons. Registration for TeamSnap will follow after the deposit window closes.

Please indicate your availability for each event listed. After the deposit window closes, we will finalize the schedule with 2 to 3 events.
