    This written warning intends to inform you of a recurring pattern of unannounced and unreported absence from work last   Pick a Date .

    In light of this violation, the following disciplinary actions will be enforced:
    1st Warning and 2nd Warning:

    • Written Warning, this incident will be formally documented in your employment file as a written warning.
    • Training and Reinforcement: You will be required to complete a refresher course to review company policies and procedures, with an emphasis on attendance and communication protocols.

    Final Warning:

    • Written Warning
    • Training and Reinforcement
    • Probationary Period: Effective immediately, you will be placed on a 90-day probationary period. During this time, you may be temporarily removed from the schedule for suspension, and any further violations could result in more serious disciplinary actions, up to and including termination.


    Failure to notify your manager is considered a no show, no call, and may lead to additional disciplinary actions, including possible termination.
