The Fall opt-in period is open September 1 until September 30, 2024.
There are no extensions to the opt-in period. Even if you experience an emergency, you will not be eligible to opt-in after the opt-in period closes. If you have any questions, need help to verify if you have been billed or not, have any issues, or need help with the opt-in process, please contact healthbenefits@lusu.ca during the opt-in period for assistance. Please contact us before the opt-in deadline to ensure that we can assist you. We will not be able to do so if the deadline has passed.
Who is eligible to opt-in?
All current Lakehead students and their dependents are eligible for the plan. The opt-in application is for:
- Students who are eligible to be billed for the plan on their tuition who wish to add dependents.
- Students not eligible to be charged for the plan on their tuition and wish to enrol into the LUSU health & dental plan.
- Students who are not eligible to be charged for the plan on their tuition and wish to enrol themselves and dependents into the LUSU health & dental plan.
If you been charged the LUSU Health and Dental Plan fee of $418.00 as indicated on your Lakehead University account statement (look for “LUSU Medical Ins” $418.00) and are not adding dependents, you do not need to opt-in as you are automatically enrolled.
When will my plan be active?
Approved applications will be processed approximately 2 weeks after the opt-in period closes. Once you are activated on the plan (approximately 2 weeks after your opt-in application is processed), you will receive an activation email that will include information and instructions on using your plan and how to make claims. Coverage will be backdated to September 1.
Are opt-in fees refundable?
Once you opt in, it's important to note that the associated fees are non-refundable. This means that once you make the decision to opt in, it is a final choice, and you won't be able to receive a refund for those fees. This includes Graduate students covered under Lakehead University Graduate Assistantship employee plan -- please ensure that you are not currently enrolled in any other GA-ship staff or an equivalent plan. Refunds will not be granted if it is later determined that you already have coverage through your GA-ship or an equivalent plan.
What does the plan cover?
Review the 2024-2025 benefits booklet here. It is important to review the benefits before submitting your opt-in application.
