My child(ren) has(have) my permission to attend all AWANA activities sponsored by PARADISE CHURCH (hereinafter the “Church”) from September 8, 2024 to May 18, 2025. I assure that he/she/they is/are in good health and able to participate. This consent form gives permission to seek whatever medical attention is deemed necessary, and releases the Church and its staff of any liability against personal losses of named child(ren). I, the parent/guardian of the above child(ren), acknowledge that participation in various events with other children necessarily involves risk of physical injury. I assume all risks and hazards incidental to the conduct of the activities/events of Paradise Church and transportation to and from all activities/events. I release, discharge, and hold harmless, Paradise Church, its staff, volunteers and any other supervisors from any claims arising out of, or relating to, any physical injury that may result to my son(s)/daughter(s) while participating in Paradise Church sponsored activities/events, including transporting my son(s)/daughter(s) to or from such activities. Photo Release:I also authorize Paradise Church to take photographs and videos of ministry activities, which may include me and/or my child(ren), and further grant my permission for that media to be utilized for public promotional purposes by the Church. This permission is valid for the dates designated on this form.
