    Dog / Puppy Adoption Application

    Happy Tails Doxies of Ky
  • Thank you for considering adoption through Happy Tails Doxies of KY!  Dogs/puppies are fun, loving, playful, and also a lot of work. Before you start this application, please ensure you can fully commit to the care of the dog/puppy before you decide to adopt one.

If you know you are ready to be a Doxie parent, please complete this application for adoption form as thoroughly as possible. The form can take up to 20-30 minutes to complete. Once submitted, someone from our team will follow-up with you soon.

(Note: We reserve the right to refuse any adoption that we feel will not suit the dog’s/puppy’s requirements.)
