Acknowledgement & Liability Waiver

I hereby consent to and authorize Jessica Cabral, Owner and Esthetician or any esthetician employed by Align Wellness Spa to perform the procedure/ treatment that I had scheduled an appointment for today.

I have voluntarily elected to undergo this treatment/procedure after the nature and purpose of this treatment has been explained to me. 

I understand and acknowledge that there are risks involved with the treatment I will be receiving. Although it is impossible to list every potential risk and complication, I have been informed of possible benefits, risks, and complications, and I have had the opportunity to ask questions regarding these risks and other possible complications

I also recognize there are no guaranteed results and that independent results are dependent upon age, skin condition, and lifestyle and that there is a possibility I may require further treatments of the treated areas to obtain the expected results at an additional cost.

I have been instructed and understand the post-treatment home care instructions. I understand how important it is to follow all instructions given to me for post-treatment care. In the event that I may have additional questions or concerns regarding my treatment of suggested home product/ post-treatment care, I will consult the esthetician immediately.

I have also, to the best of my knowledge, given an accurate account of my medical history, including all known allergies or prescription drugs or products I am currently ingesting or using topically.

I have read and fully understand this agreement and all information detailed above. I understand the procedure and accept the risks. I agree I will assume the risk and full responsibility for any and all injuries, losses, side effects, or damages which might occur to me while I am undergoing this procedure. I do not hold Jessica Cabral, Owner of Align Wellness Spa or any employee's  responsible for any of my conditions that were present, but not disclosed at the time of this skin care procedure, which may be affected by the treatment performed today.

I acknowledge that the estheticians at Align Wellness Spa may use products with natural ingredients and there is a remote chance of an allergic reaction and there is a possibility of an adverse reaction to product used in facials. Please inform your esthetican before treatment if severly allergic, as this may make you unable to receive the service. 

I am also aware that certain services should not be performed with certain medical conditions or prescriptions. I have disclosed all my known medical conditions, skin conditions, allergies, medications and answered all questions honestly on the above form and agree to update my esthetician at Align Wellness Spa as to any changes. 

I acknowledge the esthetician employee's and owner of Align Wellness Spa, Jessica Cabral, do not provide medical advice and I accept full responsibility to seek out advice before receiving any services or products from Align Wellness Spa. I hereby release, discharge and waive all claims against Align Wellness Spa and each of their employees, and representatives or any person(s) performing services or applying any products at Align Wellness Spa, including from liability and responsibility for any and all illness, injuries, damages, claims, rights and causes of action of any kind or nature, that may occur during or arising out of any services or products received on this and any future dates. I assume and accept the risk for any injuries sustained. I have read this entire document and agree to its terms.
