Please complete the following form to register for your mask fit test. Ensure that your test date is at least three weeks in advance of your placement start date. Please note that Mask Fit Testing is currently only offered at University Hospital. Kindly do your best to ensure that you are available on the day you have signed up for. Student Affairs conducts qualitative fit testing. If you are unsuccessful in completing a qualitative test, you will be referred for a quantitative fit test at a later date. *Your Transaction ID is your student number.
