This form must be submitted at least 7 days before your billing cycle date. If your account is charged, you will not be prorated any amount because we do not offer any refunds. No exceptions.

Why do you wish to terminate? *
By signing below, you agree to completely terminate your membership and forfeit any right and entitlement to the current tuition rate, any/all extension credits, and/or access to make up any missed classes. You acknowledge that this form must be submitted at least 7 days before your billing cycle date. If your account is charged, you will not be prorated any amount because we do not offer any refunds.
