Household Income Verification
As part of our application process, we require financial information from both parents/guardians to make an informed decision.
- For Parents/Guardians in the Same Household: If both parents/guardians live in the same household, we require either a tax return (IRS Form 1040) or three months of pay stubs from each parent/guardian. If one parent/guardian is not working, please submit your most recent tax return.
- For Parents/Guardians in Separate Households: If the parents/guardians do not live together, both must submit their income documentation, preferably tax returns. Include any child support or custody agreements that outline financial responsibilities for the child. If only one parent/guardian is completing the application, the other can email their documents to Scholarship@Opportunity4Kids.org with the child’s name in the subject line.

- For Parents/Guardians Who Do Not Work: If both parents/guardians are unemployed AND have not filed taxes in the past year, we require a detailed list of all government assistance received, including but not limited to SNAP, TANF, SSI, or other relevant programs. Along with these documents, please provide financial details that demonstrate your ability to cover transportation to and from the activity and any incidental fees that may arise during participation. The more transparency and information you provide us, the higher your chances of approval.

If you have any questions, please save your application and call us during business hours to discuss your options. Failure to follow these instructions will result in an automatic denial. We have limited ability to add additional documents to your child's file once the application is submitted.
