Pet Adoption Contract and Waiver

Cassidys Critters Pet Adoption Contract

As adoptive owner, I agree to the following terms and conditions: (Please Read Carefully)

I understand that it is in the best interest of myself and my other pets (if any) to take the pet, named and described on page (1) of this contract, to a licensed veterinarian within three days from the date of adoption for a routine physical examination and provide proof to Cassidys Critters. The cost of this examination is the responsibility of the adoptive owner and will not be billed to or paid for by Cassidys Critters.

1. I agree to keep the adopted pet in a healthy state by maintaining regularly scheduled visits to a licensed veterinarian for vaccinations, including rabies vaccinations according to OH law, a heartworm & flea/tick preventative, and other necessary medical needs as determined by the veterinarian. I also agree to provide documentation of vaccines and preventative to Cassidys Critters upon request, and to give Cassidys Critters permission to contact my vet to confirm vaccine history, flea/heartworm prevention & tests, and all medical treatments.
2. I agree, if not already altered, to have this pet spayed/neutered and vaccinated as defined in the "Spay/Neuter Agreement" section of this contract. I further agree if this isn't done within the time frame above a representative of Cassidys Critters has the right to reclaim the pet for the organization.
3. I agree to care for this pet in a humane manner by providing it with adequate food, water, shelter, and behavioral training if necessary. This pet is now, after all, a member of my family.
4. I agree to be a responsible guardian by providing adequate daily exercise and properly containing my pet to my property. 
5. I agree to keep a proper identification tag on my pet with my current phone number to be worn at all times. 
6. I agree that if my pet is lost, I will immediately notify Cassidys Critters by calling 330-592-5194 or emailing info to cassidycallentine@gmail.com.
7. You understand and agree once the adoption is final you are now the owner of the pet.
8. I agree I will notify Cassidys Critters within ten (10) days of a change of address so they can update the adoption record, this is so your pet can be reunited with you in the event it is lost and we are contacted.
9. I understand and agree this pet is being adopted for myself, will be kept only as a domesticated house pet, and will not be sold, bred, adopted to or given to another person or organization for any reason.
10. I understand and agree, if I cannot keep the pet, I must contact Cassidys Critters, phone 330-592-5194 or email cassidyscrittersrescue@gmail.com. Upon contact to Cassidys Critters I must advise of my inability to keep said pet and only upon written authorization from Cassidy Callentine and Cassidys Critters am I allowed to re-home said pet. If authorization to re-home is granted by Cassidys Critters I hereby agree to release Cassidy Callentine, Board Members & Cassidys Critters from liability for any injury or damages to people or property caused by the said pet, and from any causes of action, claims, suits, or demands that may arise from such injury or damages. If authorization isn't granted I must assist with the pet's return or placement to Cassidys Critters by holding the pet in a  safe environment until alternative arrangements can be made, and I will provide necessary transportation and all veterinary records to Cassidys Critters.
11. I agree to register the pet in behavioral training (if necessary) and will socialize the pet with other people & pets to ensure the pet is well adjusted to all situations. 
12. I understand and agree to allow a representative of Cassidys Critters to do a wellness check at any time to check on the welfare of the pet.
13. I agree that if Cassidys Critters does a wellness check and Cassidys Critters determines the home environment is no longer satisfactory per the organizations guidelines or no longer safe for the pet that Cassidys Critters will reclaim the pet with no refund of the adoption fee.
14. By signing this contract, I am agreeing that no one in the home has ever been investigated or charged with domestic violence, animal neglect or animal abuse and if such findings are discovered at a later date Cassidys Critters has the right to reclaim the pet with no refund of the adoption fee. 
15. I understand that if I fail to abide by any one of the terms and conditions agreed upon in this contract, Cassidy Callentine and Cassidys Critters reserves the right to reclaim the pet and terminate adoption with NO REFUND of the adoption fee.
16. Should legal action become necessary to enforce the terms of this adoption contract the adopter will pay all legal fees forCassidy Callentine and Cassidys Critters if Cassidys Critters is found to be the prevailing party in the suit.

Any and all actions to enforce this adoption contract shall take place in Summit County, Ohio and Ohio law shall apply as to the enforcement and outcome of this contract.

By signing below the Adopter agrees to all the terms and conditions within this contract.

Cassidys Critters agrees to provide Adoptive Owner with an electronic copy of this Pet Adoption Contract and medical information available for said pet.
