Our MID providers pledge to deliver the medical management of caries (MMC) and non-invasive caries therapies (NCT) in accordance with the following philosophy of care:

1. We pledge to offer MMC/NCT options to patients as the first line of treatment for dental caries.

2. We pledge to describe MMC/NCT options to patients when relevant, even if we think it might not be best in a particular situation.

3. We pledge to consider the option of no intervention generally and not the intervention we might prefer for ourselves specifically.

4. We pledge to center our practice on patient / caregiver empowerment, creating choice among interventions, and to learn from their choices.

5. We pledge to spend significant effort continuously learning about the medical management of / non-invasive therapies for oral diseases such as dental caries (MMC/NCT) and periodontal disease.

Moby Dental is a 501(c)(3) non-profit, and a small membership fee helps us cover the cost of the platform and expand our mission.

The purpose of the membership fee is to ensure the sustainability of the Moby Dental platform. Our intention is to keep the membership fees as low as possible. All applicants are provided with a 30 day trial period to account for application review and processing.  Moby Dental does not charge the membership fee until the trial period expires. 

Any provider who takes the MID pledge and is subsequently accepted into the network will be listed on Moby Dental based on the type of membership selected below:

Premium Membership at $20/mo

Includes provider profile with a digital pencil drawing photo on the "Find a Provider" tool.  The tool lists Premium Memberships ahead of Standard Memberships. 

Standard Membership at $10/mo

Includes a more basic profile on the "Find a Provider" tool with fewer data fields made available to explain your practice and services.

We feel confident that the tool will eventually drive patients to your practice who are looking for this type of care. As a non-profit, we are asking for some support from providers, but our overall funding strategy is more comprehensive.  We anticipate that the platform will find support from corporate sponsors, private grants, and donations from the general public.

Moby Dental is a 501(c)(3) non-profit and we will provide you with a confirmation that will serve as your tax deductible receipt if you decided to opt into membership. 

I affirm that I have read the MID Pledge and will or will continue to provide care in a manner that is consistent with the outlined principles.  I understand that Moby Dental reserves the right to reject applications or cancel the memberships of providers who are not demonstrating a commitment to the pledge principles as evidenced in the provider's marketing materials, website content, and patient reviews.  Moby Dental uses an established rubric to evaluate applications and monitor provider compliance.  A copy of the rubric is available upon request.

I understand that all applicants are provided with a 30 day trial period to account for application review and processing. Moby Dental does not charge the membership fee until the trial period expires.

I acknowledge and understand that I can discontinue my membership at any time.

In the event that my membership application is rejected, any rendered payments will be refunded back to my original payment method and all future drafts against that payment method will be cancelled. Rejection of applications is at the sole discretion of Moby's Executive Director.  

In the event that my profile is discontinued by Moby Dental, I understand that I will no longer be charged a monthly fee once the profile is removed.  Removal from the website is at the sole discretion of Moby's Executive Director.

I understand that Moby Dental will not provide dental or medical advice to the patients that interact with providers in the Moby Dental network. The intent of the website is to connect patients with qualified healthcare professionals.  Any question about the care rendered by a Moby Dental provider will be referred back to the rendering provider and Moby Dental will not answer questions on behalf of the provider.

I understand that the intent of the Moby Dental website is to connect patients to trusted individual providers in their local communities and the purpose of the site is not to advance the interests of any specific practice or any Dental Service Organization (DSO). Moby Dental will only list licensed providers by name and any requests to be listed as a practice will be rejected. Moby Dental does understand that most profiles will link back to practice phone numbers and websites.  Practice phone numbers and websites are permissible; however, the actual membership profile must be an individual with an appropriate license.
