PRIVACY CONDITIONS
- Access to personal information about the student may be shared between Evolution and the Australian Government and designated authorities. This information includes personal and contact details, course enrolment details and changes, and the circumstances of any suspected breach by the student.
- Personal information that has been disclosed to Commonwealth and State or Territory government departments and authorised agencies and National Centre for Vocational Education Research (NCVER) for the following purposes:

- populating authenticated vocational education and training (VET) transcripts;
- facilitating statistics and research relating to education, including surveys and data linkage;
- pre-populating registered training organisation (RTO) student enrolment forms;
- understanding how the VET market operates, for policy, workforce planning and consumer information; and
- administering VET, including program administration, regulation, monitoring and evaluation.

- The student agrees for Evolution to obtain from a credit-reporting agency containing a personal credit information about the student in relation to credit provided by Evolution.

- to assess an application by the student; and/or
- to exchange information with other credit providers as to the status of the student’s credit account, where the student is in default with other credit providers; and/or
- to assess the credit worthiness of the student.

- The student agrees for Evolution to obtain further information if necessary, from any relevant educational institution or previous employer or designated authorities as required.

 

COMPLAINT & APPEALS
- Complaints and appeals should be made and will be addressed consistently with the Grievance, Complaints and Appeals Policy.
- The right to make complaints and seek appeals of decisions and action under various processes, does not affect the rights of the student to take action under the Australian Consumer Law if the Australian Consumer Law applies.

STUDENT DECLARATIONS

By signing below, I declare that:

- the information I have provided is true and correct. I am aware of the consequences that may arise from providing false, misleading, or incomplete information, including the cancellation of my enrolment or the withdrawal of any offer made by Evolution.
- I have read, understood and will abide by the terms and conditions in this form and the Code of Conduct and Code of Practice outlined in the Student Handbook and marketing material including Course Brochures available on Evolution Website.
- I agree to be bound by Evolution’s rules and regulations, and acknowledge that all disputes arising from the details and conditions contained in the application shall be governed by and constructed in accordance with the laws of Australia and be submitted to the jurisdiction of the courts of Australia.
- I understand that I may receive an NCVER student survey.
- I authorise Evolution to obtain further information if necessary, from any relevant educational institutions or previous employers as required.
- I understand and accept that my personal information may be issued by Evolution to any Government Agency for the purposes of monitoring our RTO compliance and standards of training and assessment. The Information contained on my enrolment form may be used by my RTO for administrative, regulatory and research purposes:

- I give permission for my image, testimony or written comments to be used in Evolution's marketing material, website and social media.
- I allow Evolution to contact me regarding course requirements.
- I have read, understood, and will abide by the Assessment Conditions, and choose to be assessed at the appropriate time and place.
- my workplace has the required facilities, equipment, and resources to complete the required assessment for this qualification.
- my employer will support me with feedback and evidence for my logbook and portfolio of evidence.
- if my workplace is found to not support the training needs required of this qualification, I will not be able to complete the qualification.
- the portfolio I am responsible for collating contains my own work. I am aware of the consequences that may arise including the cancellation of my enrolment or the withdrawal of any offer made by Evolution.
- if I chose the payment plan with Direct Debit, I request and authorise Evolution Systems for Training and Development Pty Ltd to transfer funds from the account at the financial institution / Debit Card or Credit Card identified by the details I have provided, as prescribed through the Bulk Electronic Clearing System. This authorisation is to remain in force in accordance with the terms and conditions of the Direct Debit Request Service Agreement and any further instructions provided on this form. Evolution uses Stripe for payment processing solutions, ensuring that no personal card information is retained by Evolution.
