Parental Permission For Emergency Treatment

In the event of illness or accident, I give my permission for emergency treatment by qualified medical personnel for my child, and I authorize the person in charge to take my child to: Reed City Hospital.
I give consent for the facility to secure any and all necessary emergency medical care for my child.
Release of Liability

Although the safety of all activities is the primary concern, outdoor activities may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against E-Town R C and the persons in charge.

I acknowledge that the competitors and the employees of the league should be treated with respect at all times.I agree to subscribe too the leagues youtube channel.I confirm that the league and owner will not be responsible or liable if any injury or death occurs.I agree to be at every event.I am allowed to miss events for excusable reasons such as, but not limited to, work or illness as long as the following two criteria are meet.I understand that I must be respectful of everyone and that bullying of any form will not be tolerated.I understand that I will be provided with a schedule at least 60 days before the start of the season.I agree that the schedule is final and can only be changed by the owner due to incumbent weather.I consent for the league to take photographs and videos of myself at the events.I consent the league in terms of sharing those photographs and videos to social media platforms.I confirm that kids will be present in the league and at the events, therefore will refrain from using any foul language.I agree that whomever my opponent is for an event is final and cannot be changed in any way.I agree that whatever car I have for the season is final and cannot be changed in any way. And that I'm responsible for the costs for any repairs to that car throughout the season.I acknowledge that there is a non-refundable $50 fee (which can be waived by the league's owner if he finds it reasonable) if I should drop out.I have read this whole document and I accept the terms indicated above.
