We try to facilitate practice days/times according to the roster placed per team as best as we can. Sometimes, there is just no way to work around it. If there is a day you cannot do, please put that under our comments section at the bottom of this form.
What previous basketball experience has your player had?*
If they played for Club Utah, simply put CU and which team/coach.If they played for a different club/team, which club/team and what division? If they played in the Bantam League, which division? If only Rec Ball (such as Jr Jazz), just state Rec Ball. We have different levels of teams, so this will help us make evaluation more efficient. We never discriminate against experience as we have teams at every level.

Do you want your player to be considered for a travel team?*
Travel Team invites will be sent out in October. If your player will like to be considered for a Travel Team, will he/she be willing to travel? How often? Will he/she be willing to do Sunday plays? We will require the flexibility of travel players to be able to play on Sundays. (typically, playoffs are Fridays and/or Saturdays (mostly), championships are held on Sundays.) Wherever possible, if there are good tournaments that don't have Sunday play, we do try and prioritize those tournaments. We also always request that of organizers first, but rarely get that request approved. Nowadays, tournaments without Sunday play are few to none as most other state's school schedules are not as flexible as Utah's and require their students to be in school on Fridays, which does not give families much time to travel and play on Fridays. Travel players get additional and more intensive training, in addition to the training received at their local location.
Which location will you be trying out for?*
For Girls, different grades will be consolidated for tryouts in one location. This will help us create proper teams. The top priority is to create teams specific to where the player will prefer to practice out of (if you are fine practicing at a different facility, please mark that down on the following question to help us better form teams as well). If the evaluators feel strongly that your player needs to be part of a team at a different location that you specified, we will first discuss options with you before placement.

This will be for your regular local team practice and skills locations. (If you are fine to be considered for teams at different locations, please check all locations that apply.)*Travel teams may practice in a location other than their local designated location (pending on coach and team). **If your player is a better fit for a CU team at a different location, we will first discuss options with you before placement.

Please include any days/times you definitely cannot make practice (no guarantees but we try our best to accommodate players) as well as any special circumstances we should know about your player such as health issues, anxiety, ADHD, etc. This will help us know how to work with them and with you from the start. Please note here if you absolutely cannot make any tryouts or training camp dates. Recommendation is to do your best to make those dates to ensure a smooth placement.
