The Beauty of Eve Purchase Information Form

The Beauty of Eve purchase process includes a few steps:

1) SIGN USER AGREEMENT: You will receive User Agreement via JotForm. Please have the responsible party sign this document.

2) PAYMENT: You can pay by credit card and will be sent a link. If you need to make different payment arrangements, please email stlnfp@archstl.org. The cost of the Beauty of Eve program is $200. Contact the office for specific questions regarding cost, or if the cost poses a problem.

3) DELIVERY: Once the user agreement has been signed and payment has been received, we will email you a link to The Beauty of Eve Package with instructions.
