Authorization:
By registering my child for the Before School Program and/or After School Program for the 2024-25 school year, I hereby authorize Wakeman Boys & Girls Club to charge my credit card or deduct fees from my bank account for the payment of Before and/or After School Program fees. I understand the charges will be made on a recurring weekly basis according to the Club's Before and/or After School Program weekly payment schedule. I understand I am responsible for promptly updating payment information in case changes in my credit card or bank account details.
I understand insufficient funds in my bank account will result in overdraft charge of $30 per transaction.
I understand my child must have a valid active membership throughout the duration of the program. I acknowledge the Club membership fee is nonrefundable and non-transferable.
I understand BSP/ASP fees are due on a weekly basis in the week prior to participation.
I understand notice of withdrawal from any or all BSP days must be received 14 days prior to requested end date.
I understand that failure to make payment arrangements for outstanding balances may result in the suspension or termination of my Club privileges and participation in Club programs. Families are responsible for contacting Program Directors if any financial challenges occur affecting an ability to remain current on payments.
