Dear Parent/Guardian:

Your child is eligible to participate in a school sponsored activity requiring transportation to a location away from the school building. This activity will take place under the guidance and supervision of a Kalamazoo Area Mathematics and Science Center Staff member.

If you would like your child to participate in this event, please complete, sign and return this form to KAMSC. Please be advised that your student will not be allowed to participate without a signed consent form submitted to the school

Destination: West Lake Nature Preserve

We will be departing from (8:00 am) and returning to KAMSC (10:15). Students will be transported to and from the bog by licensed buses.

Method of Transportation: Bus

I consent to my child participating in the described field trip. I also consent to a staff member contacting medical help for my child, at my expense, in the event of illness or accident during the time away from school. I understand that, as parent/guardian, I remain fully responsible for any legal responsibility that may result from any personal actions taken by my child. I consent to having my child ride with an insured and licensed parent driver.

We will be outside at all times. We will be outside, regardless of the weather. Please dress appropriately for the weather. Water-proof rain boots are recommended. Please do not wear any open-toed shoes or sandals. You may want to wear sunscreen and/or insect repellent. You will need to bring a notebook and writing utensil.
