VR Cart Request Form
To request the VR cart, complete the form below and adhere to the following rules:
- Request form must be submitted at least 24 hours in advance. Any day of requests will not be honored.
- Cart should be picked up from room 213 at least 30 minutes prior to selected time.
- Cart should be returned to room 213 within 15 minutes of return time.
- Cart should be returned in the same condition it was loaned out.
- Report any issues to Ms. Lewis and Mr. Robertson through email immediately.
Failure to adhere to these rules will result in the inability to use the cart again.
