This form is not a confirmation of the appointment; Appointments will be secured once the $30-$60 deposit has been sent; This deposit will go towards the final cost of the tattoo! If you miss the appointment with no notice for rescheduling or cancel less than 24hrs in advance the deposit will be forfeit.
