I understand that by reading and signing the following terms, I will not hold Forte Dog Training, Megan Innes, or any employees of Forte Dog Training liable for any damages, loss, injury, illness, or infection sustained by myself, my dog, my agent, or anyone under my control.
I agree that the following conditions will result in the forfeiture of my position in the course and any portion of fees paid:
- Using abusive physical punishment.
- Using abusive verbal language toward any dog or person, or permitting my agent or anyone under my control to do so.
- Choosing to discontinue participation in the course for any reason once the course has begun.
- Failing to control my dog, resulting in my dog biting a person or another dog.
I understand that, in order to be registered in the class, I must submit both payment and this completed form before attending. If payment has not been submitted in advance, I understand I must provide payment by e-transfer or cash on the first day of class.
I accept full responsibility for my own dog, as well as for my guests or minors under my control. I agree to wear or bring suitable clothing and footwear for outdoor training.
I acknowledge that weather is outside the control of Forte Dog Training, and classes may be canceled or rescheduled due to poor weather conditions. I understand that no refunds will be issued less than one week prior to the start of the course.
Finally, I agree that Forte Dog Training reserves the right to use any pictures or videos taken during the course for advertising or promotional purposes.
