Exerciser hereby stipulates that he or she is physically sound and that he or she has approval to proceed with a routine of exercise and dancing provided.

Exerciser hereby also stipulates that his or her dependent child under the age of 16 years of age is physically sound and that he or she has approval to proceed with a routine of exercise and dancing provided.
LIMITATIONS OF EXERCISE, IF ANY:

It is further expressly agreed that all strength training, cardiovascular exercise, or any other exercise shall be undertaken by me at my sole risk and that the MEGA FIT CAMP event organizers and SalsaFit coaches running this program at Patricia Kemp Community Center in Vaughan, Ontario shall not be liable in case of any claims, demands, injuries, actions or causes of action, whatsoever, to my person or property arising out of or connected with the use by me of the services provided and of the premises where the same is located.

I do hereby expressly forever release and discharge MEGA FIT CAMP event organizers and SalsaFit coaches from all such claims, demands, injuries, damages, actions or causes of action, from all acts of active or passive negligence on the part of MEGA FIT CAMP event organizers and SalsaFit coaches.

I further expressly agree that I will not use equipment improperly and I give my consent so that pictures and / or videos may be taken and used to promote MEGA FIT CAMP event organizers and SalsaFit coaches.

If I have any questions whatsoever, concerning exercise and use of equipment, I agree that I will request instructions from MEGA FIT CAMP event organizers and SalsaFit coaches.

I take full responsibility and further express all actions taken by me within the premises will be done solely and based on my very own discretion and decision making and will not hold any member of coaches liable.

I hereby release responsibility of any such choices and / or consequences made by me from the Salsa Fit coaches and MEGA FIT CAMP event organizers.
